The Contractor’s application for payment shall as a minimum set out

  1. the Date;
  2. the Contractor’s name and address;
  3. the Employer’s name and address;
  4. the Contractor’s tax reference (if any);
  5. a reference to this Contract;
  6. if applicable, a reference to the Milestones achieved and the Date on which each referenced Milestone was achieved;
  7. the currency and the amount claimed in respect of each Milestone achieved;
  8. the total amount claimed for each currency.
