Must an Insurance document indicate the name of the vessel aas shown on the transport document.

No, but if a name is shown it must not conflict.

Thanks, what if the format of the insurance document indicates a space for the name of the vessel.

I would follow the principle laid down in para 38 regarding signatures i.e. the fact that a document has a box or space for a vessel name does not necessarily mean that such box or space must be completed with a name. However, if the content of a document indicates that it requires a vessel name in order to be valid it must be given.
